Other items you’ll need for your first visit:
- Your current medical insurance card(s), as well as billing information.
- Picture ID such as a drivers license.
- A list of all medications you are currently taking.
- If your insurance requires authorization for you to see a specialist, please bring an insurance referral which is separate from your insurance card. This is typically obtained from your insurance company or your primary care physician. If you have any questions or concerns about insurance or billing, please contact our patient accounts department at 425-791-3017. Our patient accounts department office hours are Monday – Friday, 8:00am – 4:30pm.
